The management of fulfilments related to Service Vouchers is carried out almost entirely in digital mode, from the Issuing Bodies reserved area.
In particular, the stages of filing rate schedules, preparing the PES, activating the Vouchers and attaching the related Time and Attendance Registers and insurance policies are completely digitised.
The following reporting documentation must be submitted to the Ad Personam Multifunctional Structure, in original:
1. In the event of a request for progress settlement
- The Schedule of the requested costs for the service provided, generated by the computer system, to which must be attached for each Voucher entered:
a) The Progress Payment Request;
b) The Report on the service provided;
c) The original Attendance Register( s) duly filled in and signed by both the Granting Body and the holder of the Service Voucher (failure to submit the original Register entails non-recognition of the conciliation services);
d) Copy of the Insurance Policy taken out.
2. In the event of a request for balance
- The Schedule of the requested costs for the service provided, generated by the computer system to which must be attached for each Voucher entered:
a) The Application for Balance Payment generated by the computer system;
b) The Attendance Register( s) in original, duly completed and signed by both the Granting Body and the holder of the Service Voucher (failure to submit the original Register entails non-recognition of the conciliation services);
c) The Report on the service provided in the terms and conditions indicated and in conformity with the PES, generated by the computer system;
d) The Report Form generated by the computer system, which gives an account of the public and private co-participation fees;
e) The Declaration on conciliation services drafted and signed by the holder of the Service Voucher, together with a copy of an identity document;
f) Copy of the Insurance Policy taken out, if not already filed and subject to its validity.
3. In the event of a Settlement Notice with a zero balance:
- The Settlement Notice with zero balance;
- The original Attendance Register(s).
