    The intervention envisages the activation of 5 Territorial Operations Centres (COTs), with the function of coordinating the taking charge of the person and the connection between services and professionals involved in the various care areas: territorial activities (home care with tracking and monitoring of the patient where the case occurs, community homes, community hospitals, etc.) health and social care activities and hospital and emergency-urgency network.

    Community Homes and Taking Care of the Person

    The intervention envisages the activation of 10 Community Homes, as an instrument for the coordination of all the health and social-health services at the territorial level through the provision of a single access point for health services in connection with the hospital network, as well as through the location of the single access point (PUA) for multidimensional assessments of social-health needs with the social and welfare services, in coordination with the initiatives proposed in Mission 5.The House will therefore constitute a continuous point of reference to ensure health promotion, prevention and care for the community of reference, especially for the chronically ill, and will also be equipped with IT infrastructure (telemedicine) and multi-specialist equipment.     Pathways to autonomy for people with disabilities

    The intervention is aimed at extending to the entire national territory the projects for "independent living" and for the "after us" experimented over the years, with attention to both the aspects linked to individualised planning, and those linked to residency and those linked to self-sufficiency through work.By the directorial decree of the Ministry of Labour and Social Policies no. 98 of 9 May 2022, the lists of projects of the Ambiti Sociali Territoriali (single ambit for the province of Trento) were approved. In this ambit, six projects were approved for a total of 4,290,000.00 euro. On 30 September 2022, the Provincial Council approved the outlines of the Agreement with the Ministry (worth approximately 2.8 million) concerning projects for the social inclusion of fragile and vulnerable subjects, and in particular for the implementation of autonomy paths for people with disabilities, in which the Apss and all the Comunità di Valle are involved with regard to the taking in charge, assessment and development of people's life projects, to be shared with the persons concerned and which, for the investment part, involve the municipalities of Torcegno, Primiero San Martino di Castrozza, Valle Laghi, Cavedine, Rovereto, Mori, Trento and Giovo.In detail below are the six projects admitted to funding:


    Project 1 within the territories of the Comunità di Valle dell'Alta Valsugana Bersntol, Valsugana e Tesino and Primiero, and in particular the Municipality of Torcegno and the Municipality of Primiero San Martino di Castrozza, owners of the buildings, with an expected number of beneficiaries equal to 5 and an expected funding of 297,516.66 euro.

    Project 2 within the territories of the Valle dei Laghi, Alto Garda e Ledro and Giudicarie Communities, and in particular the municipalities of Valle Laghi and Cavedine, owners of the buildings, with an envisaged number of beneficiaries equal to 4 and an expected funding of 237,832.47 euro.

    Project 3 within the territories of the Vallagarina Community and the Altipiani Cimbri, and in particular the municipalities of Rovereto and Mori, owners of the buildings, with an expected number of beneficiaries equal to 7 and a financing of 416,956.82 euro.

    Project 4 in the territory of the Adige Valley and the territories of the Val di Non and Val di Sole Communities and in particular the Municipality of Trento, owner of the real estate, with an envisaged number of beneficiaries equal to 12 and an envisaged funding of 714,998.00 euro.

    Project 5 within the Territory of the Adige Valley and in particular the Municipality of Trento, owner of the real estate, with an expected number of beneficiaries equal to 12 and an expected financing of 714,998.00 euro.

    Project 6 within the territories of the Community of Valle di Cembra, Val di Fiemme, Comun General di Fascia, Rotaliana and Konigsberg and Paganella and in particular the Municipality of Giovo, owner of the real estate, with an envisaged number of beneficiaries equal to 7 and an envisaged funding of 416,956.82 euro.

In this ambit, two projects were approved for a total of 420,000.00 euro. On 23 December 2022, the Provincial Council approved the outline of the Agreement with the Ministry for projects - involving all the Valley Communities - aimed at strengthening social services and preventing burn-out among operators. In detail, there are 2 projects involving all the valley communities:


    to be implemented within the Sole Valley Community, the Giudicarie Community, the Vallagarina Community and 5 territorial aggregations made up of the Rotaliana-Konigsberg (Lead Partner) and Paganella Communities, the Cembra Valley Community (Lead Partner) and the Valle dei Laghi Community, the Community of Valle di Fiemme e Fassa (Lead Partner) and Comun General del Fascia, the Community of Valsugana e Tesino (Lead Partner) and of Primiero, and the Municipality of Rovereto (Lead Partner) together with the Community of Altipiani Cimbri - with a number of beneficiaries equal to 170 and an expected funding of 210.000 euro;

    to be implemented within the territorial aggregation including the Territory of the Adige Valley, the Community of Alta Valsugana and Bersntol, the Community of Alto Garda and Ledro and the Community of Val di Non - with an expected number of beneficiaries equal to 170 and an expected funding of 210,000 euro.

    Supporting parenting skills and preventing vulnerability of families and children

    Interventions aimed at supporting parenting skills and supporting families and children in vulnerable conditions.By Directorate Decree of the Ministry of Labour and Social Policies no. 98 of 9 May 2022, the lists of projects of the Ambiti Sociali Territoriali (single sphere for the Province of Trento) were approved. In this ambit, seven projects were approved for a total of 1,480,500.00 euro. On 23 December 2022, the outline of the Agreement with the Ministry for projects aimed at supporting parenting skills and preventing the vulnerability of families and children was approved by the Provincial Council. These are projects - in which all the Valley Communities and the Municipalities of Trento and Rovereto are involved - aimed at supporting parents, in particular vulnerable families, in order to reduce the risk of children being improperly removed from their families, reinforcing the so-called P.I.P.I. Programme already active in the territory. In detail, the 7 projects concern all the Valley Communities and the Municipalities of Trento and Rovereto:


    referring to the territorial aggregation made up of the Val D'Adige Territory (lead body), the Municipalities of Trento, Aldeno, Cimone and Garniga Terme, with an expected number of beneficiaries amounting to 30 family units and with an expected funding of 211,500 euros;

    referring to the territorial aggregation made up of the Val D'Adige Territory (lead partner), the Municipalities of Trento, Aldeno, Cimone and Garniga Terme, with an expected number of beneficiaries equal to 30 households and an expected funding of 211,500 euro;

    referring to the territorial aggregation made up of the Alta Valsugana and Bersntol Community (lead partner) and the Valle di Cembra Community, with an expected number of beneficiaries equal to 30 households and an expected funding of 211,500.00 euro;

    referring to the territorial aggregation made up of the Val di Non Community (lead partner) and the Val di Sole, Paganella and Rotaliana-Königsberg Communities with an expected number of beneficiaries equal to 30 households and an expected funding of 211,500 euro;

    referring to the territorial aggregation made up of the Giudicarie Community (lead partner), the Alto Garda and Ledro Community and the Valle dei Laghi Community with an expected number of beneficiaries equal to 30 households and an expected funding of 211,500 euro;

    referring to the territorial aggregation made up of the Municipality of Rovereto (lead partner), the Vallagarina Community and the Altipiani Cimbri Community with an expected number of beneficiaries equal to 30 households and an expected funding of 211,500 euro;

    referring to the territorial aggregation composed of the Community of Valsugana and Tesino (lead partner), the Community of Val di Fiemme, the Community of Primiero and the Comun General de Fascia, with an expected number of beneficiaries equal to 30 households and an expected funding of 211,500 euro.
