Community Homes and Taking Care of the Person

The intervention envisages the activation of 10 Community Homes, as an instrument for the coordination of all the health and social-health services at the territorial level through the provision of a single access point for health services in connection with the hospital network, as well as through the location of the single access point (PUA) for multidimensional assessments of social-health needs with the social and welfare services, in coordination with the initiatives proposed in Mission 5.The House will therefore constitute a continuous point of reference to ensure health promotion, prevention and care for the community of reference, especially for the chronically ill, and will also be equipped with IT infrastructure (telemedicine) and multi-specialist equipment.
