Pathways to autonomy for people with disabilities

Mission 5 - Component 2 - Investment 1.2

Strengthening independent living experiences for persons with disabilities.

General information

The projects envisage the implementation of interventions aimed at accelerating the process of de-institutionalisation by providing home care services to people with disabilities, in order to improve their autonomy and provide them with opportunities to access the world of work, including through information technology. In particular, interventions are envisaged to renovate home spaces and/or provide ICT devices to people with disabilities, together with training in digital skills.

Specifically, there will be two lines of intervention

  1. renovation of buildings, also with the support of technology (domotisation), to make them accessible and more easily usable in the presence of disabilities
  2. strengthening of the multidisciplinary team with social and health figures (carrying out multidimensional assessment and individualised planning)
which must identify the beneficiaries and create pathways targeted to specific needs.

The interventions envisaged by the projects must be read in a future perspective, as they are aimed at guaranteeing persons with disabilities the achievement of greater autonomy and, at the same time, less dependence on the family (with a view to 'after us').

Six projects were presented in the area, referring to territorial aggregations identified on the basis of criteria such as population, territorial proximity and the distribution of health districts, which concern all the Valley Communities and the municipalities of Trento and Roveret

Goals

Enabling people with disabilities to achieve a higher quality of life through the implementation of pathways to independence.

One of the results to be achieved is the consolidation of the experimental paths already started through the social housing and independent living projects.

Target participation: for the 6 projects implemented at provincial level, at least 47 beneficiaries are expected to be involved.

Monitoring

By Directorate Decree of the Ministry of Labour and Social Policies no. 98 of 9 May 2022, the lists of projects of the Ambiti Sociali Territoriali (single framework for the Province of Trento) were approved.

On 30 September 2022, the Provincial Council approved the outlines of the Agreement with the Ministry concerning the projects for the social inclusion of fragile and vulnerable subjects, and in particular for the implementation of autonomy paths for people with disabilities, in which the Azienda Provinciale per i Servizi Sanitari and all the Comunità di Valle are involved with regard to taking charge, assessing and drawing up life projects for people, to be shared with those concerned and which, for the investment part, involve the municipalities of Torcegno, Primiero San Martino di Castrozza, Valle Laghi, Cavedine, Rovereto, Mori, Trento and Giovo.

In 2022, directives were also adopted to the APSS aimed at strengthening the Multidisciplinary Disability Assessment Unit and the supporting information systems (Resolution no. 2469 of 22 December 2022) and the agreement for the implementation of the projects was signed between the Province, the Community identified as lead partner, the Municipalities owning the buildings and the other Communities (Resolution no. 2429 of 22 December 2022).

In 2023 priority interventions were identified for the implementation of public housing renovation and the provision of domotic equipment for disabled users (Resolution no. 710 of 28 April 2023)
