Community Homes

The Community Centre serves as a permanent point of reference to ensurehealth promotion andprevention, as well as the provision of care for the local community, particularly for those with chronic conditions; it is also equipped with IT infrastructure (telemedicine) and a range of specialist medical equipment. It serves as a single point of access to local health and social care services, in coordination with the hospital network, with the aim of providing local services to residents.

The organisational model, already in place in the area, has been further strengthened through structural investments which have, to varying degrees, involved existing facilities (similar in function to the ‘Case della Comunità’) as well as newly built facilities designated for this purpose.

To provide an integrated response to basic healthcare needs, with ongoing points of contact for the population, in order to ensure thecare of the local community, including through a blood-taking centre, a range of specialist and basic diagnostic equipment, and an IT infrastructure.

Target: a minimum of10 Community Centres, whilst also ensuring energy efficiency.

The Autonomous Province of Trento, in its capacity as the implementing body, utilisesthe Integrated University Health Authority of Trentino (ASUIT) as its delegated implementing body.

The implementation of the PNRR in Trentino fits seamlessly within the provincial framework for sustainable development, in line with the policies set out in the Provincial Economic and Financial Document and the related Update Note.

The project has met and exceeded its target of 10 Community Centres, with 11 facilities now operational in the areas ofAla, Cles, Borgo Valsugana, North Trento, Malé, Sen Jan di Fassa, Riva del Garda, Rovereto, Pergine Valsugana, Mezzolombardo and South Trento.

The projects carried out have helped to strengthen the local healthcare network by upgrading and adapting existing facilities and establishing new centres, equipped with services and resources to facilitate integrated care and localised treatment.

Today, the Community Centres serve as key points of reference for accessing health and social care services, helping to strengthen local care provision and continuity of care, particularly for people with chronic conditions.
