The form, which must indicate the name of each travelling attraction for which registration is requested, must be signed in front of the official receiving the application or delivered by hand or posted, accompanied by a photocopy of an identification document of the applicant and a number of € 16.00 revenue stamps (one to be applied to the application + one for each attraction to be registered).
If the applicant is a non-EU citizen, a photocopy of a valid residence permit must also be produced.
In the case of an application transmitted via an e-mail box (certified or simple), the applicant must sign it, either with a digital signature or with a handwritten signature; in the latter case, after signing it, he/she must scan it ("scanning it") and transmit it together with a copy of an identity document. The revenue stamps must be affixed to the application and cancelled; alternatively, the details of the revenue stamps (day and time of issue, 14-digit identifier) may be indicated on the application.
- 2 copies of the activity booklet for each attraction (if the application is submitted via the municipality there must be 3 copies);
- 2 copies of the operation and maintenance manual of the activity for each attraction (if the application is submitted via the Municipality there must be 3 copies);
- valid annual inspection certificate for each attraction;
Please note that the inspection is subject to prior payment of € 200.
This payment shall be made
- either by direct payment to the treasury account of the Autonomous Province of Trento, exclusively at the counters of the Treasurer in charge of the Pat - UniCredit S.p.A., specifying the following data: BANK code (5079), ENTE code (400), and, as reason for payment, "Sopralluogo Commissione provinciale di vigilanza" (Provincial Supervisory Commission inspection)
- or via the pagoPA platform - the national node for electronic payments:
- directly online; it is necessary to connect to mypay.provincia.tn.it, select the entity "Provincia Autonoma di Trento"; enter the "Codice Avviso" (unique code for the individual payment indicated in the last line of the notice) and finally choose the preferred payment channel;
- alternatively, you can show the notice for digital payment at bank counters, Sisal and Lottomatica circuits (tobacconists) or at other counters and/or channels enabled to PagoPA.

Form no. 004438 (COMMUNICATION for change of ownership of the attraction)
The form, which must indicate the name of each travelling attraction for which the change of ownership is requested, must be signed in front of the official receiving the request or delivered by hand or sent, accompanied by a photocopy of an identity document of the applicant (the person transferring, the person taking over).
There is no stamp duty for this communication.
If the interested parties are non-EU citizens, a photocopy of their valid residence permit must also be produced.
