Licence application
Forms and related documentation may be delivered by hand, mailed or sent by certified mail to the following address: serv.polamm@pec.provincia.tn.it.
If the application is not produced by hand, it must be accompanied by a photocopy of an identification document of the applicant. If the applicant is a non-EU citizen, a photocopy of a valid residence permit must also be produced.

- No. 2 € 16.00 revenue stamps (one stamp must be affixed to the application)
- Form no. 004479
- Table of operations, signed by the owner, with the relevant fee schedule, duly completed;
- Photocopy of the plan , signed by the owner, of the premises where the activity is carried out, with the identification data noted (necessary in the case of transfer of the activity, extension/reduction of the surface area allocated to the activity);
- Register (business journal), pursuant to Article 120 of the T.U.L.P.S., which may also be kept in computerised form, for endorsement pursuant to Article 16 of the T.U.L.P.S. Regulations, if the activity is carried out in the Municipality of Trento;
(N.B.: for activities carried out in the remaining municipalities, the register must be presented to the local P.S. Authority, i.e. to the Commissioner of P.S. for Rovereto and Riva D/G, to the Mayor in the remaining municipalities);
- Form no. 004473 (DICH2)
To be produced only if the owner does not complete the last point of the declaration contained in the SCIA (the one following the table showing the names of the partners).
Declaration in lieu of certification and affidavit completed by each partner, accompanied by a photocopy of an identification document of the signatory. If this is a non-EU citizen, a photocopy of a valid residence permit must also be produced.

Declaration in lieu of certification and affidavit completed by the representative (if communicated), accompanied by a photocopy of an identification document. If this is a non-EU citizen, a photocopy of a valid residence permit must also be produced.
- Form no. 004476 (DICH4)
Declarations in lieu of certification and affidavit filled in by the agents (if communicated), with the relevant photocopies of identity documents and, if non-EU citizen, also photocopies of residence permits or residence cards.
All criminal records must be indicated in the declarations; even those
- not impeding the exercise of the activity;
- not resulting from the criminal record certificate issued to the interested party pursuant to Presidential Decree 313/2002;
- applied pursuant to Article 444 of the Code of Criminal Procedure (application of the penalty at the request of the parties);
- criminal decrees of conviction.
- Original of the licence in case of request for variations.
Only the documents, listed above, relevant to the application must be attached to it.
Substitute declarations are subject to random checks, as well as to compulsory spot checks in all cases where there are reasonable doubts as to the truthfulness of the declarations made or where elements are brought to light that indicate the vagueness of the situation described.
