T007 - Duplicate vehicle registration certificate for theft/loss/destruction;

T003 - Duplicate vehicle registration certificate for deterioration.

• form TT2118 completed and signed by the holder;
• police report in case of loss/theft/destruction or vehicle registration certificate if deteriorated;
• copy of a valid identity document of the holder;
• in case of proxy, attach Form 2118 - Proxy.
