Grants for the funding of sports solidarity (application)

This grant is awarded to cover the costs incurred for rehabilitation and specialist care at facilities accredited by or affiliated with the National Health Service, including those across borders, as well as for the purchase of assistive devices, where these are not already provided for under current health or social care legislation.

Each beneficiary may submit multiple applications over several years, up to the amount specified in Article 28 of Provincial Law No. 4/2016 and within the limits of the resources available in the provincial budget.

The first application must be submitted within three years of the date of the report certifying permanent civil disability issued by the Forensic Medicine Unit of the Provincial Health Services Agency of the Autonomous Province of Trento.

Eligible expenses, inclusive of VAT, are those relating to:
a) rehabilitation;
b) specialist care;
c) hospitalisation for rehabilitation and/or specialist care and for respite care;
d) the purchase of direct and indirect aids.

The expenses described above must be consistent with the care plan drawn up for the patient by the Provincial Health Services Authority of the Autonomous Province of Trento and must have been incurred after the submission of the initial application; they must be clearly attributable to the disabling event and must not already be covered by the National Health Service, the social care service or sports insurance.

Solidarity funding is granted at 100 per cent of eligible expenditure, up to the limit set out in Article 28 of Provincial Law No. 4/2016, amounting to 50,000 euros.

The grant is provided for under Article 28 of Provincial Law No. 4 of 21 April 2016, ‘Provincial Law on Sport 2016’.

Athletes resident in the province who have permanent physical disabilities resulting from injuries sustained whilst practising a sport governed by sports federations recognised by CONI and CIP operating at provincial level in the amateur sector.

The application must be signed by the applicant or by a person legally authorised to act on behalf of the beneficiary of the grant, such as a guardian, curator, support administrator or other such figure.

The application must be submitted using the editable form provided by the local authority. It may be sent in PDF or JPG format to the certified email addressserv.sport@pec.provincia.tn.it.

Should the available funds be exhausted, applications shall remain valid for the following financial year.

1. A fully completed, dated and signed application form.
2. A certificate confirming the disabling sports injury.
3. A prescription for the necessary aids and care requirements, issued by a doctor from the National Health Service.
4. A full expenditure plan including cost estimates, dated and signed. For applications following the first one, a full expenditure plan including cost estimates or invoices if the expenses have already been incurred.
5. A declaration that the expenses covered by the application are not covered by other grants, insurance or compensation.
6. Privacy notice pursuant to Articles 13 and 14 of EU Regulation No 679 of 2016, dated and signed.
7. A photocopy of the applicant’s identity document, if the application is signed by hand on a scanned document and not in the presence of the relevant staff member.

For applications subsequent to the first one, the documentation referred to in point 2 need no longer be attached.

Applications may be submitted at any time of the year.
