Contributions towards the funding of sports solidarity schemes (settlement)

This is the application submitted by beneficiaries who have been granted funding to cover the costs incurred for rehabilitation and specialist treatment at facilities accredited by or affiliated with the National Health Service, including cross-border care, as well as for the purchase of assistive devices, where these are not already provided for under current health or social care legislation.

Solidarity funding is granted at 100 per cent of eligible expenditure, up to the amount set out in Article 28 of Provincial Law No. 4/2016, namely 50,000 euros, and in any event within the limits of the budget deficit and the grant awarded.

The grant is provided for under Article 28 of Provincial Law No. 4 of 21 April 2016, ‘Provincial Law on Sport 2016’.

Athletes resident in the province with permanent physical disabilities resulting from injuries sustained whilst practising a sport governed by sports federations recognised by CONI and CIP, to whom the requested funding has been granted by decision of the head of the provincial body responsible for sport.

The application must be signed by the applicant or by a person legally authorised to act on behalf of the beneficiary of the grant, such as a guardian, curator, support administrator or other such figure.

The application for payment of the contribution must be submitted using the editable form provided by the administration and sent as a static PDF or JPG file to the certified email addressserv.sport@pec.provincia.tn.it.

1. An application form completed in full, dated and signed.
2. A statement of expenses incurred, dated and signed.
3. A list of expense documents on the provided form, in both PDF and spreadsheet formats, dated and signed.
4. Copies of invoices marked as paid, or other documentation of equivalent probative value, relating to all expenses listed in the statement of expenses incurred.
5. Information notice pursuant to Articles 13 and 14 of EU Regulation No 679 of 2016, dated and signed.
6. A photocopy of the applicant’s identity document, if the application is signed by hand on a scanned document and not in the presence of the relevant member of staff.

The application for payment of the grant must be submitted by the deadline set by the head of the provincial department responsible for sport in the decision granting the funding.
