Information, instructions, regulations and forms for submitting the application for a contribution by means of an assessment procedure to cover the expenses for rehabilitation and specialist activities as well as for the purchase of aids by athletes residing in the province of Trento with permanent physical disabilities resulting from injuries occurring during the practice of sporting activities.

Description

This is the application submitted by beneficiaries who have obtained funding to cover the expenses incurred for rehabilitation and specialist activities at accredited facilities or facilities affiliated with the national health service, including cross-border facilities, as well as for the purchase of aids, when they are not already provided for by current health or welfare legislation.

Solidarity funding is granted to the extent of 100% of the eligible expenditure, up to the amount defined in Article 28 of Provincial Law no. 4/2016, equal to 50,000 euros, and in any case within the limit of the deficit and the contribution granted.

The contribution is provided for in Article 28 of Provincial Law No. 4 of 21 April 2016 "Provincial Law on Sport 2016".

Restrictions

The application for payment of the grant must be submitted within the deadline set by the manager of the Tourism and Sport Service in the decision granting the grant.

Who it's for

Athletes residing in the province with permanent physical disabilities resulting from injuries occurring in the practice of sporting activities regulated by the sports federations recognised by CONI and CIP to whom the requested funding has been granted by order of the Head of the Tourism and Sport Service.

The application must be signed by the applicant or by the person legally authorised to act on behalf of the grantee as guardian, curator, support administrator or other figure.

The application for the grant must be submitted on the fillable form prepared by the administration and sent in static PDF or JPG format to the certified mailbox serv.turismo@pec.provincia.tn.it, or delivered by hand to the Sports Activities Office.

Documents to be submitted

1. Application completed in full, dated and signed.
2. Statement of expenses incurred, dated and signed.
3. List of expenditure documents on prepared form, either in PDF or spreadsheet format, dated and signed.
4. A receipted copy of the invoices or other documentation of equivalent probative value relating to all expenses set out in the statement of expenses incurred.
5. Information pursuant to Articles 13 and 14 of EU Regulation No 679 of 2016, dated and signed.
6. Photocopy of an identity document of the applicant, if the application is signed with a handwritten signature on a scanned document and not in the presence of the employee in charge.

Settlement of the grant awarded is effected upon conclusion of the assessment of the accounts submitted, with a payment order to the current account indicated in the application. If supplementary documentation is requested, the time limit for the proceedings is suspended until it is received.
