Information, instructions, regulations and forms to submit an application for a contribution by means of an assessment procedure to cover the expenses for rehabilitation and specialist activities as well as for the purchase of aids by athletes residing in the province of Trento with permanent physical disabilities resulting from injuries occurring during the practice of sporting activities.

Description

Funding is granted to cover the expenses incurred for rehabilitation and specialist activities at accredited facilities or facilities affiliated with the national health service, including cross-border facilities, as well as for the purchase of aids, when they are not already provided for by the health or welfare regulations in force.

Each beneficiary may submit several applications over several years, up to the amount defined in Article 28 of Provincial Law no. 4/2016 and within the limit of the resources available in the provincial budget.

The first application must be submitted within three years from the date of the report establishing the state of permanent civil invalidity by the Operational Unit for Forensic Medicine of the Provincial Agency for Health Services of the Autonomous Province of Trento.

Expenditure, including VAT, is eligible for:
(a) rehabilitation activities
b) specialist activities;
c) hospitalisation for rehabilitation and/or specialist activities and for respite services;
(d) purchase of direct and indirect aids.

The expenses described above must be consistent with the health path defined for the patient by the Provincial Board for Health Services of the Autonomous Province of Trento and must have been incurred after the submission of the first application, must be clearly attributable to the disabling event and may not already be covered by the National Health Service, the Welfare Service or sports insurance.

Solidarity funding is granted to the extent of 100% of the eligible expenditure, up to the amount defined in Article 28 of Provincial Law No. 4/2016, equal to 50,000 euros.

The contribution is provided for in Article 28 of Provincial Law No. 4 of 21 April 2016 "Provincial Law on Sport 2016".

Restrictions

The application may be submitted at any time.

Who it's for

Athletes resident in the province with permanent physical disabilities resulting from injuries occurring in the practice of sporting activities regulated by the sports federations recognised by CONI and CIP operating at provincial level in the amateur sector.

The application must be signed by the applicant or by the person legally authorised to act on behalf of the beneficiary as guardian, curator, support administrator or other figure.

What to do

The application must be submitted on the fillable form provided by the administration. It can be sent in PDF or JPG format to the certified mailbox serv.turismo@pec.provincia.tn.it, or delivered by hand to the Sports Activities Office.

Special cases

If the available resources are exhausted, applications remain valid for the following financial year.

What you need

Documents to be submitted

1. Application completed in full, dated and signed.
2. Certification attesting to the disabling sports injury.
3. Prescription of the necessary aids and assistance needs, by a doctor from the public health service.
4. Complete, dated and signed expenditure plan. In the case of applications subsequent to the first application, complete cost plan with estimates or invoices if expenses have already been incurred.
5. Declaration that the expenses claimed are not covered by any other contribution, insurance or indemnity.
6. Information pursuant to Articles 13 and 14 of EU Regulation No 679 of 2016, dated and signed.
7. Photocopy of an identity document of the applicant, if the application is signed with a handwritten signature on a scanned document and not in the presence of the employee in charge.

Applications subsequent to the first must no longer be accompanied by the documents referred to in point 2.

Timeframes and deadlines

Maximum no. of waiting days

The 120-day period for the conclusion of the procedure starts on the day following the submission of the application. If additional documentation is requested, the time limit for the proceedings is suspended until it is received.
