The projects envisage the implementation of actions to strengthen social services in the home to ensure early discharge and prevent hospitalisation, favouring a unitary and multi-professional social and health care takeover, paying specific attention to increasing the skills of operators.
Each project will be divided into three integrated actions
- activation of home care services to guarantee the LEPS (Essential Levels of Services in the Social Field) "protected discharges", to be activated in synergy with the Provincial Agency for Health Services, with the aim of reducing hospitalisation times
- activation of training courses for the professionals involved in "protected discharge" to guarantee uniformity of service;
- activation of home care services to supplement the LEPS, ensuring/increasing the current offer of social services in favour of home care.
In particular, the recipients of the services activated, reported by the hospital network and assessed by a multidisciplinary team, will be able to benefit from a social and health care plan to guarantee continuity of care at home or in another suitable place for the homeless.
The territories to be involved in each project were identified taking into account the criteria defined at ministerial level and the territorial configuration of Spazio Argento, as set out in the resolution of the Provincial Council no. 1719/2022. The resources were allocated on the basis of the total resident population and the population over 65.
By means of Directorial Decree of the Ministry of Labour and Social Policies no. 98 of 9 May 2022, the lists of projects of the Ambiti Sociali Territoriali (single framework for the Province of Trento) were approved.
In 2023 the outlines of the agreement between the Province and the local level implementing bodies were approved (Resolution no. 1762 of 29 September 2023).
Goals
Preventing the aggravation of highly fragile situations that would lead to institutionalisation of the elderly person, by activating home care services to favour early discharge or prevent, where possible, hospitalisation.
Target: at least 250 beneficiaries (125 beneficiaries for each project), vulnerable persons without a formal support network (also homeless persons), in the process of being discharged from hospital services, who need a residential context of protection and support.
In addition, training courses will be set up for at least 60 professionals in the social, health and socio-medical fields.
