Home-based social services

The projects involve measures to strengthen home-based social services in order to facilitate early discharge and prevent hospital admission, promoting a unified, multi-professional social and healthcare approach, with a particular focus on enhancing the skills of care workers.

Each project will be structured around three interlinked actions:

• the roll-out of home care services to ensure compliance with the LEPS (Essential Levels of Social Services) ‘protected discharge’ scheme, to be implemented in collaboration with the Provincial Health Services Authority, with the aim of reducing the time spent in hospital;
• the roll-out of training programmes for professionals involved in ‘protected discharges’ to ensure consistency of service;
• the roll-out of home care services to complement the LEPS, ensuring and expanding the current provision of social services to support care at home.

In particular, those referred to the services by the hospital network and assessed by a multidisciplinary team will be able to benefit from a social and healthcare support plan designed to ensure continuity of care at home or in another suitable location for people experiencing homelessness.

The areas to be included in each project have been identified taking into account the criteria defined at ministerial level and the territorial configuration of Spazio Argento, as set out in Provincial Council Resolution No. 1719/2022. Resources have been allocated on the basis of the total resident population and the population aged over 65.

ByDirectorial DecreeNo. 98 of 9 May 2022 of the Ministry of Labour and Social Policies, the lists of projects for the Territorial Social Areas (a single area for the province of Trento) were approved.

In 2023, the draft agreements between the Province and the local implementing bodies were approved (Resolution No. 1762 of 29 September 2023).

To prevent situations of severe vulnerability from worsening – which would lead to the elderly person being placed in care – by mobilising home care services to facilitate early discharge or, where possible, prevent hospitalisation.

Target group: at least 250 beneficiaries (125 beneficiaries per project), vulnerable individuals who do not have a formal support network (including people experiencing homelessness), who are in the process of being discharged from hospital and who require a residential setting offering protection and support.

In addition, training programmes will be organised for at least 60 professionals working in the social, health and social-health sectors.

The Autonomous Province of Trento is the implementing body, acting as a single territorial unit, with the involvement of local authorities and the Provincial Health Services Agency as sub-implementing bodies.

The implementation of the PNRR in Trentino forms an integral part of the provincial policy framework for sustainable development, in line with the policies set out in the Provincial Economic and Financial Document and the related Update Note.

PNRR funding: €660,000.00 (€330,000 for each project)

As at 30 June 2026, in accordance with the set deadline,theproject’stargethad been met and exceeded.

A total of 326 beneficiaries were reached, and more than 60 professionals from the health and social care sectors were trained.
