General information

The projects involve measures to strengthen home-based social services in order to facilitate early discharge and prevent hospital admission, promoting a unified, multi-professional social and healthcare approach, with a particular focus on enhancing the skills of care workers.

Each project will be structured around three interlinked actions:

  • the roll-out of home care services to ensure compliance with the LEPS (Essential Levels of Social Services) ‘protected discharge’ scheme, to be implemented in collaboration with the Provincial Health Services Authority, with the aim of reducing the time spent in hospital;
  • the roll-out of training programmes for professionals involved in ‘protected discharges’ to ensure consistency of service;
  • the roll-out of home care services to complement the LEPS, ensuring and expanding the current provision of social services to support care at home.

In particular, those referred to the services by the hospital network and assessed by a multidisciplinary team will be able to benefit from a social and healthcare support plan designed to ensure continuity of care at home or in another suitable location for people experiencing homelessness.
