Community Homes

The Community Centre will serve as a permanent hub to ensure the promotion andprevention of ill health and the provision of care for the local community, particularly for those with chronic conditions, and will also be equipped with IT infrastructure (telemedicine) and a range of specialist medical equipment. It will serve as a single point of access to local health and social care services, working in coordination with the hospital network to ensure local services for residents.

The organisational model, already in place in the area, will be further strengthened through structural investments that will affect, to varying degrees, existing facilities (similar in function to the Community Centres), as well as facilities that will be used for this purpose, including newly built ones.

Community Centres are planned for Ala, Cles, Borgo Valsugana, North Trento, Malé, Sen Jan di Fassa, Riva del Garda, Rovereto, Pergine Valsugana, Mezzolombardo and South Trento.

To provide an integrated response to basic care needs, with permanent points of contact for thelocalpopulation that ensurethe community is properly looked after, including through a blood sampling centre, a range of specialist and basic diagnostic equipment, and an IT infrastructure.

Target: a minimum of10 Community Centres, whilst also ensuring energy efficiency.

The Autonomous Province of Trento, in its capacity as the implementing body, has appointedthe Trentino Integrated University Health Authority (ASUIT) as its delegated implementing body.

The implementation of the PNRR in Trentino is fully aligned with the provincial policy framework for sustainable development, in accordance with the policies set out in the Provincial Economic and Financial Document and the related Update Note.

PAT funding: €7,056,954.12

The target has been met for the Community Centres in Ala, Malé, Mezzolombardo, Trento Sud, Rovereto, Riva del Garda and Borgo Valsugana.

For the remaining Community Homes in Sen Jan di Fassa, Pergine Valsugana, Trento Nord and Cles, the procedures are currently being finalised and are expected to be completed shortly.
