General information
The Community House will be an ongoing point of reference to ensure health promotion, prevention and care for the community of reference, especially for the chronically ill, and will also be equipped with IT infrastructure (telemedicine) and multi-specialist equipment. It will be a single access point for health and social-health services in the area, therefore, in coordination with the hospital network, with the aim of guaranteeing proximity services to residents.
The organisational model, which is already present in the area, will be further strengthened with structural investments that will involve, to varying degrees, existing structures (similar in function to the Community Homes), as well as structures that will be used for this function, including newly built ones.
Community Houses are planned in Ala, Cles, Borgo Valsugana, Predazzo (with provincial resources), Trento nord, Malé, Sen Jan di Fassa, Riva del Garda, Rovereto, Pergine Valsugana, Mezzolombardo and Trento sud.
Goals
To provide an integrated response to basic care needs, with continuous points of reference for the population to ensure that the community is taken care of, also by means of a collection point, basic multi-specialist and diagnostic equipment, and an IT infrastructure.
Target: at least 10 Community Houses (3 to be built, 7 to be renovated), also ensuring energy efficiency.
