Description
It is a tool designed to accompany the person with disabilities along his or her life path, with the aim of guaranteeing the full realisation of the person, improving the quality of life and ensuring the greatest possible autonomy. It enhances the desires, needs and potential of the person with disabilities, involving the services and the community to enable the person to define and realise his or her own life goals.
What the Life Project includes
It is a written document, which sets medium/long term objectives, identifies the resources available for their realisation, taking into account
- the person's wishes and aspirations: what they want to do, where they want to live, etc.
- objectives to be achieved: living alone, doing sports, finding employment, etc. To check the achievement of objectives, indicators are identified
- necessary supports: the list of all interventions, services and benefits (public and private) needed to achieve the objectives
- economic resources: through the project budget, the resources, both public and private, including those of the person and the family, destined for the project are identified (human, professional, technological, instrumental and economic resources, which can also be activated within the territorial community and the informal support system)
To whom it is addressed
All citizens with disabilities recognised following the basic assessment carried out pursuant to Legislative Decree 62/2024, by the Operative Unit of Legal Medicine of ASUIT or in possession of certification pursuant to Law 104/1992 (issued by 31/12/2026).
Actors involved:
- Person with disabilities and/or their representative
- Family/caregiver, other figures, if involved by the person
- TheMultidimensional Assessment Unit (UVM): the team of professionals who, taking into account the person's wishes and aspirations, assess resources, abilities, needs and support requirements
- The project implementation contact person: the reference figure for the implementation of the Project identified in the UVM
- Socialservices, health services, schools, employment centres and the Third Sector, private subjects, informal resources, depending on the project's objectives.
How it is done
The person with disabilities is the Owner of the Life Project, which is developed through a participatory process, which includes the following phases
- Request: the person with disabilities or who represents him/her requests the activation of the Project to the Social Service of the Valley Community where he/she resides.
- Multidimensional Assessment: the UVM is formed, which comprehensively assesses the situation.
- Drafting: on the basis of the assessment, the Project is written together with the person with disabilities.
- Appointment of the contact person for the implementation of the Project: a professional is identified who will be the point of reference for the implementation and coordination of all the services.
- Implementation andMonitoring: the Project is put into practice, it is periodically monitored and updated if the person's situation, needs and choices change.
What is needed
The Life Project application can be presented by the person with disabilities or whoever represents him/her, at any time and in free form, at the Social Services of the Valley Community of residence or of the Municipalities of Trento or Rovereto.

The application can be submitted at any time.
The maximum waiting days are 120.
