General information
The structuring of a multidisciplinary team to take charge and the identification of a specialist case manager is envisaged for persons potentially in need of an accompaniment pathway (social, work, housing, access to low-threshold services, etc.) because they are in the legal condition to be admitted to alternative measures to detention or are close to the end of their sentence. This action contained in the 2024-2026 Action Plan"For the social reintegration of persons in penal execution and for the development of restorative justice paths" concerning the area "Social reintegration, family ties and culture" will be formalised with the signing of an operational protocol between the parties.
Goals
The action tends towards real care of persons subject to measures restricting their personal liberty by defining
- of the composition, functions and working methods of the multidisciplinary team
- of the criteria for identifying the case manager;
- of the operating methods and relations between the different actors involved;
- the procedures for customising social reintegration programmes.
Milestones
- 12/04/2024 Establishment of the working group coordinated by the A.P.A.S. ODV as the managing body of the provincial service "Social inclusion of persons subjected to measures restricting personal liberty";
- 31/12/2024: signing of the operational protocol.
