Students who have lost or damaged their vocational qualification or vocational diploma certificates obtained at an educational institution of the Autonomous Province of Trento

What to do

In order to obtain the replacement certificate, an application must be made by filling in the appropriate form on plain paper (under forms below).
The application should then be sent

or hand-delivered to the Ufficio Istruzione e formazione professionale - IeFP room no. 4.29 - Servizio Formazione Professionale, formazione terziaria e funzioni di sistema, via G. Gilli, 3 - Trento

What you need

Documents to be submitted

The application must be accompanied by a copy of an identification document (identity card, driving licence or any other document proving the applicant's identity), duly signed information pursuant to Articles 13 and 14 of EU Regulation No. 679 of 2016 and a copy of the report of the loss report issued by the competent authorities.
