Licence application for advertising concerning spas

Applications may be submitted by spas

Apply for authorisation on the appropriate form (code 10565, approved by determination of the Director of the Health and Non-self-sufficiency Policies Service no. 8288 of 21 December 2021), at any time during the year, accompanied by a €16.00 revenue stamp, to the pec address serv.politsanitarie@pec.provincia.tn.it.

- Application for licence for advertising concerning spas (form 10565 approved by determination of the Director of the Health and Non-self-sufficiency Policies Service no. 8288 of 21 December 2021);

- Information pursuant to Articles 13 and 14 of EU Regulation No. 679 of 2016 on the processing of personal data duly signed;

- Photocopy of an identity document of the applicant (if the application is signed with a handwritten signature and not in the presence of the employee in charge);

- The scientific text that will appear in the advertisement;

- text relating to the prevention campaign (specify which one if the case);

- other types of documentation to be advertised.

The application may be submitted at any time of the year.
