The following must be attached to the request:
- privacy policy pursuant to Articles 13 and 14 of EU Regulation No. 679 of 2016, signed for receipt and acknowledgement;
- documentation proving the state of economic difficulty
- photocopy of an identity document of the applicant (if the request is signed with a handwritten signature and not in the presence of the employee in charge).
Formats of the documentation transmitted electronically
When transmitting the documentation electronically, the files must have one of the following formats (and extensions): PDF/A, PDF, XML, TXT, TIFF, TIF, JPEG, JPG, JFIF, JPE and EML, BMP, CSV and GIF. The HTML format is also permitted, limited to the body of the e-mail message. Compressed formats (e.g. .zip) are not accepted, as the unambiguous identification of compressed and/or enveloped files is not guaranteed either by the certified e-mail service or by the document management system.
