Driving licence - Duplicate theft loss

Within 48 hours of the loss, theft, or destruction of a driving licence, the holder of the licence must report it to the police, who will simultaneously issue a provisional driving licence. From the moment of issuance of this provisional permit, the licence identified in the report is no longer valid, and in the event of its discovery or return, the complainant is obliged to destroy it.

Once the denunciation has been made, the Police authority will inform the person concerned whether he will receive the duplicate directly at home, by the Central Operations Office (C.O.) of the Ministry of Transport and Navigation, or whether he will have to go to the Provincial Motor Vehicle Office to apply.

The user may go to the Provincial Motor Vehicle Office to apply for a duplicate of the lost, stolen or destroyed driving licence only in two cases

• if he has been referred by the authority that received the complaint, the latter having ascertained that the duplicate document cannot be extracted from the national register of driving licence holders
• if it has been addressed by the Central Operations Office, the latter having ascertained that the duplicate cannot be extracted from the national register of driving licences.

The service is open to all interested parties.

• make an appointment at the offices of the Civil Vehicle Registration Authority, via the 'digital channels' section;
• complete and submit the documents listed under "Documents to be submitted".

The PAGOPA code associated with the tariff for "Duplicate driving licence for theft loss" is:

• N007 "Duplicate driving licence for loss or theft" (NATIONAL AMBICTION);
• T007 'Duplicate driving licence for loss or theft (non-duplicable UCO)' (TRENTO AMBIT).

NB: Payment N007 is made in the hypothesis that the licence is duplicable. Payment T007 in the hypothesis that the licence is non-duplicable.

In the event that the lost driving licence cannot be duplicated, the file must be submitted to the Driver and Vehicle Licence Office by submitting the following documents

• complete form TT2112;
• present the provisional driving licence and the report of loss/stolen/destruction issued by the Police authority;
• present two recent passport-size photographs on a white background and bareheaded on non-thermal paper, signed on the back (if the application is not presented directly by the person concerned, one of the photos must be legalised at the local authority) (it is possible to use the self-service kiosk for passport photos available at the vehicle registration office)
• present a valid identity document;
• if the file is not presented by the applicant, form TT2112 (signed by the applicant) must be accompanied by a proxy on plain paper and a photocopy of an identity document of the licence holder; the proxy must in any case present a valid identity document
• receipt of payment made with PAGOPA of tariff T007 "Duplicate driving licence for loss, theft and destruction" (AMBITO TRENTO).
• if the driving licence is about to expire, it is advisable to submit the dematerialised medical certificate issued by the authorised doctor (renewal will take place at the same time); in the same way, one must proceed if the licence has already expired. In both cases, form TT2112 should be submitted requesting a duplicate for loss with simultaneous renewal, with payment of the fee T038 - DUPLICATE FOR LOSS WITH CONSEQUENT RENEWAL (RIGHTS € 10.20 | POSTAGE € 32.00) instead of the fee T007 mentioned above.

N.B: Non-EU citizens must show their valid residence permit in original.
