Driving licence - Duplicate for deterioration

If your driving licence has deteriorated, you can apply for a duplicate at the Driver and Vehicle Licensing Agency (DMV) offices.

The service is open to all interested parties.

• make an appointment at the offices of the Civil Vehicle Registration Authority, see section "Digital Channels";
• complete and submit the documents listed in the "Documents to be submitted" section.

The PAGOPA code associated with the tariff for "Duplicate driving licence for deterioration" is:

• T003 "Duplicate driving licence for deterioration"(AMBITO TRENTO).

NB: Before proceeding with payment, check that you have selected AMBITO TRENTO and not NAZIONALE.

• complete form TT2112;
• present two recent passport-size photographs on a white background and uncovered head on non-thermal paper, signed on the back (if the application is not presented directly by the person concerned, one of the photos must be legalised at the local authority) (it is possible to use the self-service kiosk for passport photos available at the motor vehicle registration office)
• if the driving licence is about to expire, it is advisable to present the medical certificate on stamps issued by the authorised doctor (renewal will be carried out at the same time)
• present a valid identity document;
• attach the deteriorated driving licence;
• if the application is not submitted by the person concerned, form TT2112 (signed by the applicant) must be accompanied by a proxy on plain paper and a photocopy of an identity document of the licence holder; the proxy must in any case produce a valid identity document;
• receipt of payment made via PAGOPA.
