Documents to be submitted

Holders of driving licences issued by an EU Member State:

  • complete form TT2112;
  • the original foreign driving licence and two photocopies;
  • two recent passport-sized photographs on a white background, with head uncovered, on non-thermal paper, signed on the back
  • complete the form ‘substitute declaration in lieu of a notarial act and certification’ certifying the date of first residence in Italy;
  • a medical certificate with a photograph, issued by an authorised doctor, and a photocopy thereof (only if the applicant wishes to obtain a new period of validity for their driving licence or in the case of conversion of driving licences with no expiry date or with an expiry date exceeding that provided for by the current EU directive);
  • a valid identity document;
  • receipt of payment made via PAGOPA;
  • if the application is not submitted by the applicant themselves, a power of attorney on plain paper and valid identity documents, either originals or photocopies, of both the person granting the power of attorney and the person acting on their behalf.
