Authorisation to carry out transport activities for the sick

Persons intending to carry out nursing transport activities.

Apply for authorisation on the appropriate form (code 10567, approved by determination of the Director of the Health and Non-self-sufficiency Policy Service no. 8288 of 21 December 2021), at any time of the year, to the pec address serv.politsanitarie@pec.provincia.tn.it.

- Application for authorisation to carry out the activity of nurse transport (form 10567, approved by determination of the Director of the Health and Non-self-sufficiency Policies Service no. 8288 of 21 December 2021);

- Information pursuant to Articles 13 and 14 of EU Regulation No. 679 of 2016 on the processing of personal data duly signed;

- Photocopy of an identity document of the applicant (if the application is signed with a handwritten signature and not in the presence of the employee in charge);

- List of vehicles used (type, manufacturer, number plate, year of registration);

- Name of the medical director and his/her declaration of acceptance of the appointment;

- Planimetry of the premises with indication of the intended use of the premises.

The application may be submitted at any time of the year.
