Description
Reintegration after the therapeutic community is a delicate phase, often marked by housing and relational difficulties, with the risk of relapse. For this reason, accompaniment, support and continuity of care are essential.
Social reintegration facilities organised as collective family residences play a fundamental role, as they accommodate up to 6 persons who have successfully completed the community pathway. The objective is to consolidate personal autonomy and foster social and work integration (LEA art. 35, c.2, lett. c).
Reception envisages an individualised project, shared with the service network, and a temporary stay defined by the UVM, as the final stage of the therapeutic-rehabilitation pathway.
