Authorisation for activities to ascertain fitness for competitive sport

Private entities wishing to carry out assessment and certification of fitness for competitive sport (doctors specialising in sports medicine)

Apply for authorisation on the appropriate form (code 10561, approved by the determination of the Director of the Health Policies and Non-self-sufficiency Service no. 8288 of 21 December 2021), at any time during the year, accompanied by a revenue stamp of € 16.00, to the pec address serv.politsanitarie@pec.provincia.tn.it (for information contact Alessandro Granello 0461-493814 or Elena Mengon 0461-494213).

Any cessation of activity or any change concerning the certifying doctors, the location or the ownership of the facility must be communicated to the Health and Non-self-sufficiency Policies Service.

- Application for authorisation for the activity of ascertaining and certifying fitness for competitive sports (form 10561, approved by determination of the Director of the Health and Non-self-sufficiency Policies Service no. 8288 of 21 December 2021);

- Information pursuant to Articles 13 and 14 of EU Regulation No. 679 of 2016 on the processing of personal data duly signed;

- Photocopy of an identity document of the applicant (if the application is signed with a handwritten signature and not in the presence of the employee in charge);

- List of the equipment used to carry out the activity;

- planimetry of the premises on a scale of 1:100, showing the intended use in duplicate.

The application can be submitted at any time of the year to the Health and Care Policies Service.
