Apply for inclusion on the appropriate form (code 10559, approved by determination of the Director of the Health and Non-self-sufficiency Policies Service no. 8288 of 21 December 2021), at any time during the year, at the pec address serv.politsanitarie@pec.provincia.tn.it.

- Application for inclusion in the list of border pharmacies authorised to dispense medicines to residents in the Province of Trento (code 10559, approved by determination of the Director of the Health Policies and Non-self-sufficiency Service no. 8288 of 21 December 2021);

- Information pursuant to Articles 13 and 14 of EU Regulation no. 679 of 2016 on the processing of personal data duly signed;

- Photocopy of an identity document of the applicant (if the application is signed with a handwritten signature and not in the presence of the employee in charge).
