Request for accommodation in the facilities managed by Opera Universitaria di Trento for doctors specialising at Universities affiliated with the Autonomous Province of Trento during the training network period at the facilities of the Azienda sanitaria universitaria integrata del Trentino (ASUIT).

Fill in the form, have the confirmation section signed and stamped by the ASUIT traineeship and internship contact person and send it, together with
