Mission 5 - Component 3 - Investment 1.1.2 - Neighbourhood health facilities

The intervention aims to consolidate the role of rural pharmacies in municipalities with less than 3,000 inhabitants, as structures capable of providing territorial health services in order to better cover the range of health services offered to the population of inland areas (understood as that part of the territory characterised by a significant distance from the centres offering essential services).

Description

The grants are intended for the implementation of various types of interventions, including the reorganisation and implementation of the dispensing area and the storage of medicines, participation in the care of chronic patients, including through home care projects, and the strengthening of telemedicine services.

Thanks to the interventions financed by the call, rural pharmacies will be able to

- participate in the integrated home care service, sharing the taking charge of the chronic patient, increasing the rate of patient adherence to pharmacological therapies and monitoring the correct use of medicines

- provide second-level services, through diagnostic-therapeutic pathways designed for specific pathologies;

- dispensing drugs that patients are now obliged to collect from the hospital;

- monitoring patients with the electronic medical record and the pharmaceutical dossier.
